Passengers under five years of age and crew members are exempt from this procedure. All passengers will also be required to undergo a temperature check. E TICKETS
The new electronic form replaces the Traveler's Health Affidavit, Customs Declaration and International Boarding/Disembarkation Card forms. Passengers must complete an arrival and departure form and the system will generate two QR codes. The E-Ticket can be completed prior to travel or before passing through Immigration at the Dominican airport. We recommend completing the forms 72 hours prior to travel, print or take a screen shot of the QR code and have it on hand during arrival for the Customs authorities to scan. The QR code will not be scanned during departure, but it is a confirmation that the form was completed correctly. If you need to make a change to the form you will need to complete it again.
